Maybe the answer is obvious, but physics was not my strong suit... should the plane be in level flight attitude when you balance the elevators? Or does it matter?

I'm not sure exactly what you're asking? If you add the recommended weight of lead to the right area you can't go too far wrong.
The elevator should have the same weight on either side of the hinge point, once complete. That includes everything down the fabric and paint. To achieve that, you need to estimate the weight of the fabric and paint on the main elevator, and overweight the horn to prepare for the weight your add later.
Once balanced, it will be balanced about the hinge line in any rotation / attitude.
