In the United States we have the NASA ASRS system which you can access at https://asrs.arc.nasa.gov/
For users outside of the US, it is likely that your government sanctions a similar system. If you make an error, you can file a report with the ASRS and under most circumstances, be immune to punitive certificate action from the FAA. There are some exceptions, such as cases involving intentional acts, criminal activity, and lack of qualification. Consult an attorney if the consequences are high.
